Application

Applications are accepted from January 1st through the last day of February of every year. If you submit an application outside of that window, your application will not be reviewed.

To apply, complete the application below, then email the following to UEI@hsc.utah.edu:

• A copy of your résumé;
• two letters of recommendation, either academic or professional;
• a clear, current picture of yourself; and
• a transcript (official or unofficial) from the most recently attended educational institution demonstrating a minimum GPA of 3.2. This is required for admission to UEI. Any exceptions will be considered by the UEI Board of Directors when extenuating circumstances exist.
